Adobe Acrobat 7.0 Reader to open the file. Below is a link for a free copy of Adobe Acrobat 7.0 Reader. Then fax the completed form back to HIRE-SAFE at: 866 619-1050
2. We will then email back your Subscriber ID number and all the needed compliance and disclosure forms.
3. Distribute the supplied disclosures to your applicant, and have them sign the Release Authorization.
4. Then FAX to our office only the signed Release Authorization: 866 619-1050
