Transportation Background Check: $49.00 A broad-based search that will eliminate those who do not meet the minimum standards and TSA requirements. This SEVEN POINT profile package includes:
- Social Security Number Validation
This search will examine the applicant’s Social Security number and confirm that it is a valid match to the subject name.
- Address History
The address history of the applicant from the previous seven years is revealed.
- Alias Names Listed
- Commercial Driver’s License Information System (CDLIS)
The CDLIS search will satisfy the FMCSA requirement to obtain a 3-year driving history on regulated prior commercial driver’s licenses.
- GlobalScan Terrorist Watch List Report
 This federal watch-list screening of high-risk persons is provided for compliance with The USA Patriot Act of 2001.
- Motor Vehicle Reports (State MVR)
The applicant’s domicile state driving record will be pulled, revealing any moving violations.
- National Criminal Profile Report*
This initial criminal record search is researched through a broad proprietary database containing more than 245 million criminal records. This information is compiled from a variety of sources: state and county criminal record repositories; prison, parole and release files, state agencies and Sex Offender Registries, where available. There are NO database searches that contain sufficient information for employment or tenant suitability hiring decisions for California based companies and residents. ALL CRIMINAL RECORDS FOUND MUST BE FIRST VERIFIED BY COUNTY COURT RECORDS PRIOR TO TAKING ADVERSE ACTION. The employer is legally responsible for violations of this state law, and fines can be heavy.

Release of Information Form - - 49 CFR Part 40 Drug & Alcohol Testing
DOT Mandated Drug Testing To facilitate Department of Transportation mandated drug testing, HireSafe has partnered with Quest Diagnostics and LabCorp, providing over 13,000 collection sites nationwide. Remote accident location and after-hours services are available as needed.

The requirements for DOT drug testing differ from non-DOT in the following:
49 CFR Part 40 (DOT drug and alcohol testing regulations) These regulations define the reporting requirements and responsibilities of the MRO and C/TPA as well as the responsibilities of the Employer DER (Designated Employer Representative).
DATIA NAADATP accreditation Applicant Insight is one of only 3% of TPAs nationally that is certified by the Drug & Alcohol Testing Industry Association. As outlined by the accreditation standards, Applicant Insight has requirements and responsibilities that go beyond Part 40 into administration and operational procedures.
Applicant InsightT America will operate as both the Medical Review Officer (MRO) and the Third Party Administrator (TPA) in accordance with federal regulations. The administrative details are:
1. Applicant Insight will report results directly to the Designated Employee Representative (DER).
2. Acting as the TPA, Applicant Insight will be able to work directly with the client (including random selection communication).
3. A signed Client Services Agreement (CSA) outlines our services and responsibilities.
4. Invoicing will be handled by HireSafe.
Some additional information: The DER is a person at the employer, generally a supervisor, that can remove employees from safety-sensitive positions should they test positive, etc. HireSafe only coordinates DOT testing for the following DOT testing modes: FAA, FMCSA, FTA, and PHMSA. HireSafe will coordinate DOT testing programs for "owner operators," which would be a person who owns his or her own semi and works as an independent shipping contractor. Most employers that must comply with DOT regulations are covered by the Federal Motor Carriers Safety Administration (FMCSA) and eligibility is dependent on whether the employee is driving a CMV (Covered Motor Vehicle).
Random Program Coordination: Random testing is a requirement of all DOT Model Agencies. The rates differ by Agency and can be found on the DOT Website. To set up the program, a list of eligible employees and their SSN’s are provided to HireSafe. Lists are emailed in password-protected Excel format to the DER and alternate contact(s).
The Random Testing Rate for Federal Motor Carrier Safety Administration (FMCSA) is:
- Drug Testing at 50%
- Alcohol Testing at 10%
