Drug Testing Collection Site Locations
When your drug testing account was setup your Patient Service Center (PSC) collection site was provided to you based upon your location. Either LabCorp or Quest Diagnostics was assigned to your account and the appropriate Custody Control Forms (CCF) were sent to you. These forms are NOT interchangeable between the two different PSC’s. If a different PSC is requested please contact HIRE-SAFE and new CCF’s will be sent to you.

- Select "Occupational Urine Drug Screen Collections"
- Enter the zip code OR the city and state where the applicant is located
- "Find PSC" for a listing of collection sites. Increase the radius to find additional sites, if appropriate in rural areas. Maps or driving directions can be obtained by clicking on the appropriate links.

Understanding Third Party Fees On the Patient Service Center Results Search Page, only the collection sites that begin with “Quest Diagnostics” are laboratory Patient Service Centers (PSC’s). All other locations listed are “Third Party Collection Sites” and subject to additional collection fees.

When visiting a Patient Service Center, individuals should bring the following information with them:
- The Laboratory Custody Control Form (CCF) provided to you by HIRE-SAFE. Please be certain the laboratory name on the CCF matches the PSC you are going to.
- A photo ID for urine drug screens (e.g., driver's license, employee identification badge).
